Job Description & Requirements - Specialty: Physical Therapist - Discipline: Therapy - Duration: 13 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Travel Job Title - Physical Therapist - SNF/LTC Location - Libby, Montana Schedule / Job Type - Full-time – 40 hours per week, on-site Key Responsibilities: - Develop and implement personalized treatment plans to restore function, improve mobility, and relieve pain for residents. - Assess patient needs, set achievable goals, and modify care plans under physician guidance. - Collaborate with interdisciplinary teams to evaluate progress and adjust therapy strategies. - Educate residents and their families on therapy methods and progress. - Document patient assessments, treatment plans, and progress in compliance with regulations.
