Job Description:
Description The Care Manager RN is responsible for managing
patient populations with chronic illness and/or multiple
co-morbidities. This position works in coordination with multiple
PCPs and serves as the central link to an identified population of

we must empower them. Required Qualifications: Coursework/Training:
Graduation from an approved, accredited school of Nursing.
Washington Registered Nurse License upon hire. 2 years of work
experience in an RN role. Work experience revolving around
medications and IV solutions in terms of purpose, administration,
side effects and contraindications and administers according to
established standards and provider orders. Experience managing
complex patients in an ambulatory setting. Preferred
Qualifications: Bachelor's Degree in Nursing. Why Join Providence?
